RN Breast Navigator-BCNSummary/Objective
The RN Breast Navigator is responsible for coordinating the
provision of patient care services for all newly diagnosed breast
cancer patients referred to the Baptist Cancer Network Breast
Concierge Program.
Essential Job Duties

1. Provides and reinforces education to patients, families, and
caregivers about diagnosis, treatment options, post-treatment care
and survivorship. Orients and educates patients, families, and
caregivers to the Baptist Cancer Network, multidisciplinary team
member roles, and available resources. Assesses and updates patient
education programs and resource tools as needed.
2. Collaborates and facilitates communication among members of the
multidisciplinary cancer care team to prevent fragmented or delayed
care that could adversely affect patient outcomes.
3. Facilitates timely scheduling of appointments, diagnostic
testing, and procedures to expedite the plan of care and to promote
continuity of care.
4. Assesses patient needs upon initial encounter and periodically
throughout navigation, matching unmet needs with appropriate
services and referrals and support services, such as dietitians,
providers, social work, genetic counseling, chaplain, clinical
trials and financial services
5. Assists with multidisciplinary planning conferences, develop
concise patient summaries, and presents case summaries at the
multidisciplinary planning conference.
6. Participates in the tracking of metrics and patient outcomes, in
collaboration with Director, Clinical Manager and Oncology
Informatics Nurse, to document and evaluate outcomes of the Breast
Concierge Program.

Physical Demands

required to stand and walk. The employee is occasionally required

may need to be able to assist Participants.
Required Education and Experience
Associates Degree
1-3 years of related experience.
Required License/Certifications
Current Texas RN License and Breast Navigation Certification within
a year of hire.
